Canyon Lake Dental & Orthodontics complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, sex, age, or disability. We do not exclude anyone, deny anyone care, or treat anyone differently on any of those grounds.
Help with a disability, free of charge
Tell us what you need. We will provide reasonable modifications and appropriate communication aids at no cost where required — for example, accessible formats or help communicating with the practice. If something about the office or website is a barrier, please call us so the practice can arrange an appropriate option.
Help with language, free of charge
If English isn't your first language, say so when you call and we will arrange an interpreter at no cost to you. You don't have to bring a family member or a friend to translate, though you're welcome to have someone with you if you'd rather.

If you believe we have discriminated against you
Please raise it with us first, and we will look into it. Ask for Dr. Elizabeth Pohlmann, Owner and Privacy Officer, by phone or in writing at the practice. You can ask for our grievance procedure, and you can bring someone with you.
You do not have to come to us first, and you can do both. You have the right to complain directly to the U.S. Department of Health and Human Services, Office for Civil Rights. File online at ocrportal.hhs.gov, or get the complaint form, and the address to mail it to, at hhs.gov/ocr/complaints. If you need help filing, the Office for Civil Rights will help you.
Nobody is treated differently at this practice for having made a complaint, and nobody ever will be.
