This page explains what happens to information you send through this website. It is separate from our Notice of Privacy Practices, which covers your health records and your rights over them.
This site sets no cookies
There's no cookie banner on this website because there's nothing to consent to. We set no cookies, we run no analytics or advertising trackers, and we don't build a profile of you. The fonts and images are served with the site rather than through advertising or analytics services. Like any web host, Cloudflare processes ordinary request data such as your IP address, browser information, the page requested, and the time of the request in order to deliver and protect the site. We do not add tracking scripts to that activity.
What the contact form collects
When online callback requests are enabled, the form asks for your name, a phone number, an optional email address, a general office category, and when you'd rather be called. It does not include a message box. We use those details only to respond to your request.
Your browser sends the form directly to a separately hosted AWS service; it does not send the form body through Cloudflare Pages. AWS API Gateway, Lambda, and Amazon SES process the request and deliver it to the practice inbox. That service may be enabled only after the AWS account is covered by a Business Associate Agreement and the endpoint has passed its privacy and security checks.
The website does not put your submission in a site database or keep a second copy. The email received by the practice remains in its managed inbox according to the practice's retention and records obligations. The AWS service is configured not to log the form body; ordinary security and delivery metadata may still be retained by the service providers.
Please don't describe symptoms in the form
The callback form deliberately has no free-text field and its categories do not ask what is wrong. Please give symptoms, diagnoses, and other clinical details by phone or in person, not through this form or ordinary email.
Who sees it
AWS processes the request to deliver it to the practice's managed inbox. It is then read by authorized practice staff who need it to respond. We do not sell form information or share it for advertising.
Links to other sites
Some links here lead elsewhere — to Google Maps for directions, or to our listing there. Once you follow one of those you're on someone else's website, under their privacy policy rather than ours.
Children
We treat children as patients, but this website isn't aimed at them, and the form's meant to be filled out by a parent or guardian.
Asking us about your information
You can ask what the practice holds about you or ask for a correction. A request to delete information may be limited by health-record, legal, or operational retention duties. Call the practice, or write to the office address in the footer, and ask for the privacy contact.
